In order to adequately assess your current condition our clinician will need the full picture of your current status. 


We will want to review:

1. Any prior doctors visit notes that pertains to your condition and symptomps

2. Imaging reports such as MRIs, CTs or Xrays that may have been done

3. Physical therapy notes and/or notation of any home exercise programs that you have or currently practice


We recommend that you put in a request with the physician currently overseeing your care to please send this information to our office at fax. 614-956-7011 or email Referrals@pssmohio.com

- Ohio Health and Mount Carmel can send to us through their EMR - as a partner we have access to see their referrals through their EPIC system. 


We ask new patients to arrive to our office 30 minutes early with their photo ID and insurance card to help patient intake run smoothly!

Our New Patient paperwork is listed on our Patient Information page of the website, can be filled out in office or through the self-check in feature of our portal. 


Please come to your appointment with your case manager's information, such as name and direct phone number as well as email if they have one available. This helps with smooth coordination of your care and any needed BWC authorization requests that will be completed in our office. 


We are a very small office, with an intimate number of providers and staff, all of which having been with us for several years, up to 14 years. We have consciously structured it this way for maximum efficacy in our ability to all work seamlessly together in the collaboration and coordination of your care.

Yes! Please eat prior to the procedure -- NPO (withholding food/water) is not required.


If you take a prescription blood thinner medication, please double check with our office at 614-591-0020 to see if you need to stop or temporarily hold the blood thinner prior to your procedure.


For your first initial injection we do recommend bringing someone who can drive you home after. 


Do not immerse the injection site in water for 24 hours following the procedure. For pain management, use cold packs and ice; do NOT use heat the day of the procedure. You may resume normal activities the following day.

Telehealth Flexibilities extended through Dec 2027

 Medicare patients can receive telehealth services for non-behavioral/mental health care in their home through December 31, 2027 .
